I tried an experiment once with a home of eight Sims with random assorted traits (and just one bathroom, because it was amusing).
If I hadn't stepped in, she would have stood in place until she had an "accident" and then finally ate when the bar got full red, then had to shower. Last night I had a Sim get hungry, make some food, put it down, decide she needs to go to the bathroom, left the food, then canceled the action to go to the bathroom, then stood and complained about both until I manually directed her to eat and use the toilet. Especially right now with some of the bugs in the games.
I've tried to let them run their own lives at times.
